When private healthcare information is required by the participant, all records are kept confidential to the consulting team of My Learning Well. If authorization is required to discuss your goals or concerns with other authorities, you will receive a pdf form to fill in and send back via email. Outside contacts for records or additional information will not be accessed without your full cooperation and permission.

By accessing our website and materials associated with it, you agree you are accepting the terms of this agreement and you agree to release and discharge My Learning Well owners and coaches from any and all responsibilities or liabilities. If you, the client, have any questions about the services, background information of your consultants, or strategies used, we will make every effort to accommodate your request to assure clarity between you and My Learning Well PRIOR to services rendered.

By agreeing to services, you are also agreeing to our Privacy Policy. We are also bound to the rules and regulations asserted by the National and Connecticut Code of Professional Responsibility for Teachers and Administrators which can be found at www.ncate.org and www.ct.gov/sde/cert. You are expected to review these if there are any questions regarding standards or confidentiality of My Learning Well providers and our services.

Once again, if you have any questions about the services, background information of your consultants, or strategies used, we will make every effort to accommodate your request to assure clarity between you, the client, and My Learning Well PRIOR to services rendered. Clients are encouraged to ask for clarification at any time, however, we are not responsible for facilitating the questioning process after the initial consultation.
